Order number containing the item you wish to lodge a complaint about, or the invoice number
Item number of the item you wish to lodge a complaint about, or a precise description
Grounds for complaint (Ideally you should include a short description here about why you want to lodge a complaint about the item. For example, "The seam is split on the inside of the right shoe" or "The jacket's zip is broken", etc.)
If possible, please include a photo of the item
